How could the credit bureau(s) report me deceased?

Some malicious person/ probably an ex spouse possibly sent a death certificate through my old creditor to only 2 of 3 major credit bureaus. How can I legally make them send me an explanation of how/where they got this information in order to fight them. Can they just claim you dead without any legit information [...] http://bit.ly/oai5gY
